Mother and Newborn Tragically Pass After Home Birth

A mother and her newborn daughter tragically passed away within a short span after a home birth, as revealed during an inquest. Jennifer Cahill, aged 34, lost her life the day following the birth of her second child, Agnes. Sadly, Agnes survived for only four days before also succumbing. The heartbreaking events occurred on June 3, 2024, at the family residence of Jennifer and Rob Cahill in Prestwich, Bury, in the Greater Manchester area.

Hospital staff, following the incident, mentioned that opting for a home birth went against recommended practice and advice, citing concerns stemming from Jennifer’s prior childbirth. Nevertheless, Rob stated that these warnings were never explicitly communicated to them. Testifying in court on Monday, Rob recounted the harrowing moment when he had to dial 999 around 6:45 am on June 3, as baby Agnes was born unresponsive and resuscitation efforts were unsuccessful. He then rushed Agnes to the hospital, only to discover that his wife, described by her family as exceptionally kind, was also being transported to the hospital due to birth complications.

The inquest, conducted at Rochdale Coroner’s Court, has yet to disclose the causes of death for Jennifer and Agnes. Rob emotionally shared the poignant experience of holding Agnes for her first and final birthday celebration before bidding her farewell. Reflecting on Agnes, he expressed gratitude for the time spent with her, providing comfort amidst the tragic circumstances.

Caroline Nixon, the community midwife closely involved during Jennifer’s second pregnancy, admitted in court that a referral to a senior midwife should have been made due to Jennifer’s high-risk status. This acknowledgment raised the possibility that Jennifer might have reconsidered a home birth.

Jennifer, originally from Worcester, met her husband during her final university year in Manchester in 2011. They tied the knot in 2018 and welcomed their first child, Rudy, in 2021. Anticipation for Rudy’s sibling was met with tragedy, as Jennifer faced postpartum complications following Rudy’s birth, necessitating a blood transfusion due to substantial blood loss and potential transmission of infection to her newborn.

Due to these health complications, Jennifer was referred to a consultant at North Manchester General Hospital, under the management of Manchester University NHS Foundation Trust (MFT), to establish a tailored delivery and treatment strategy for her second child. The plan recommended a hospital birth with active management, including specific interventions to mitigate potential risks during delivery.

Despite these recommendations, Jennifer opted for a home birth, citing concerns stemming from her previous traumatic hospital birth experience. Community midwife Ms. Nixon acknowledged the deviation from the initial birthing plan and highlighted the necessity for a detailed discussion on the associated risks, especially concerning transferring to a hospital in case of emergencies.

During the inquest, it was revealed that concern among community midwives had been expressed regarding high-risk pregnancies and home births. The emphasis was on respecting women’s choices, though the potential risks and implications were not always explicitly discussed. The court also learned that the phrases “out of guidance” and “against medical advice” were not commonly used in discussions, as midwives focused on supporting women’s decisions.

Rob disclosed that his wife’s medical records did not indicate any “out of guidance” warnings concerning her choices or birth plan. Conversations between Jennifer and Rob post-doctor appointments primarily centered on her status as a strep B carrier, rather than the risk of postpartum hemorrhage.
